    Please use this form to upload your applications. **DO NOT FORGET TO HIT SUBMIT AT THE END OF THIS FORM. Any incomplete applications will receive an email denial with reason. The license itself cannot be completed until the criminal background check has been completed, the Common Council has approved the license, and all fees have been paid. Note: if there are any outstanding debt due to the City or the County, you may not be approved until those debts have been paid in full. Complete payment should be mailed to the Clerk's office at 500 Genesee St. Delafield, WI 53018. TO PAY BY CREDIT CARD, USE THE LINK ON THE SUBMISSION CONFIRMATION PAGE.

    The City would like to collect the payment for your license in full at the time of the application. We understand we cannot make that a condition of your license. It does however speed up the process by which you receive your license - which will be issued via email.
  8. Premise Description Information

    The City requires a detailed premises description. Please fill out the following fields. What you list here will appear on your license. Please review your current posted license for the existing premises information. Include any additional areas being utilized this year, and exclude any areas no longer in use. This description must conform with your City of Delafield Business Plan of Operation.

  13. NOTE: If your business has a drive up service that you wanted to include as part of alcohol sales, please include this in your premise description. Your business plan of operation must include the drive up portion. Check with the City Planner if you are unsure.

    By submitting this document, you acknowledge the following: I understand I must pay the license fee in full if I wish to have a liquor license issued to my business. I understand I can have no outstanding debt with the City of Delafield and possibly Waukesha County. I understand the agent information is sent to the Police Department for a criminal background check, a legal notice for the license is placed in the paper, and the Common Council must approve the license application before the Clerk can issue me my license. If I hold a reserve license, I understand my license is not eligible for a premises-to-premises transfer. I understand I cannot sell my retail alcohol license to another person or entity, that they must apply through the City and I either relinquish/surrender my license within the year or I do not apply for a renewal of the license at the end of the license period.
  18. Upon issuance by the City Clerk, physical licenses will be emailed to the address listed earlier in this form. Paper licenses will not be mailed except upon written request. Licenses must be printed and displayed so they are visible from the point of sale.
